Only if you do it wrong, or turn the power up too much. Otherwise, done properly it is a real buzz. Literally, it buzzes your cock or balls or whatever you connect to. I have done it for many years.
Whit stepped up training it does not hurt at all. With E-stim you can turn up the current as high or low as you like. However taking care that you do not get "burned".
